How to Register for a Mining Course

For any Mining Inductions/courses simply contact OHSA direct via phone. email or fax to book your place.

What information do I supply when registering for a Mining Induction

We require your name,residential and postal address, email address, most relevant phone number and your drivers licence number. Please have your credit card handy as all courses must be paid in full upon registration. Payment options are listed below

How to Register for OHSA Courses other than Mining Inductions

OHSA Course Registration Form

Once you have selected your course/s, venue/s and date/s you can either contact us to register yourself and secure that place for your chosen training program . info@ohsa.com.au or call our office direct, on 1300 647 200 or simply download the Registration form located at the bottom of this page. Complete the form and print, sign your name where indicated and then either fax back to us on F:1300 852 970 or email to us via courses@ohsa.com.au Other courses require that you download, complete and return an Enrolment form to OHSA via fax or email:

Payment

Full payment is required before commencement of training, unless prior arrangements have been made with OHSA Occupational Health Services Australia management or accounts department.

Full payment for courses must be made when submitting your registration request, invoices will be issued to persons/organistaions who are paying on behalf of students and must be paid before a student is officially registered.

Pay by credit card

If you would like to pay by credit card please complete and return a Credit Card Authority form, which is available below. Return along with your Course Registration /Enrolment Form .Alternatively, you can contact our accounts department on 1300 647 200 and we will be able to process your payment.

Pay by direct credit

When paying by direct credit you must quote the Master ID name you have provided on the credit card Authority form or the Invoice number located at the top right hand corner of your invoice. Failure to quote either of these numbers may result in your payment not being able to be recognised against your account.

Proof of Identity Requirement

Participants in accredited training modules must provide proof of identity to the responsible person at OHSA Occupational Health Services Australia, who will sight and verify the evidence on the first day of training, record the type of evidence provided on the attendance sheet and sign the declaration. Details of appropriate forms of identification are available below.
